Complex Bileaflet Mitral Valve Repair (Barlow's) Using The Da Vinci Robotic Surgical System

OBJECTIVE:This video presents a complex repair of mitral valve with a bileaflet prolapse (Barlow's) using the da Vinci Robotic Surgical System (RMVR).
METHODS:A 50-year old male presented with severe mitral regurgitation. Transesophageal echocardiography (TEE) revealed multiple regurgitant jets with bileaflet prolapse and annular dilatation. Left ventricular function was preserved.
RESULTS:The patient underwent RMVR consisting of P2 quadrangular resection with sliding plasty, transfer of chords from P2 to A2/A3, closure of cleft A3/P3 and P2/P3 and ring annuloplasty. Postoperative TEE demonstrated a competent mitral valve. Patient had an uneventful recovery and was discharged home on the third postoperative day
CONCLUSIONS: RMVR can be safely used to repair mitral valves with complex pathology such as bileaflet prolapse using conventional techniques of repair.
